The failure occurred on Southwest Airlines flight 1380 on April 17, 2018, after takeoff from New York City's LaGuardia Airport. Fragments from the engine struck the plane's fuselage, causing rapid depressurization, and a female passenger died as a result. Eight others suffered minor injuries.
After traveling about 55 miles, it was deliberately derailed by a company control center in Perth, according to ATSB.

A new Highway Code from England's Department for Transport will help keep cyclists and pedestrians safe on the roads, Transport Minister Jesse Norman announced recently. The government’s review follows the release of road casualty figures showing that 101 cyclists died in 2017 in road traffic collisions.
"These advanced applications show the potential for data-driven safety solutions that the department wants to support," said Under Secretary of Transportation Derek Kan.

"A solution to reverse this trend is to give drivers more control over their own schedules."
Applicants will compete for awards of $7.5 million for each center, and applicants must submit a required letter of intent by Oct. 12, 2018.
