Among the violations is one for not having “an adequate fall protection system.”
The company faces citations for two willful, four repeat and seven serious violations.
Siteworks Inc. faces proposed penalties of $85,005 following the inspections.
An employee allegedly worked at the location for more than a month in these conditions, OSHA said.
The discount retailer’s string of citations continues with proposed penalties of $171K after a recent inspection.
Before these citations, two workers died from falls, one in 2019 and one more recently in 2022.
Each contractor faces at least one serious citation.
The discount retailer is now facing proposed penalties of $294K.
OSHA also proposed penalties of $46,875 following inspections in Colorado, Idaho and New York.

The guidance, announced in late January, will take effect on March 27.
