Contractor Dies Following 18 Foot Fall at Surface Hoist Facility

A miner died from injuries sustained after falling through an unguarded opening on the fifth floor of a surface hoist structure.

Safety officials are reporting the death of a contractor miner following an 18-foot fall through an opening at a surface mine facility.

The accident occurred on the fifth floor of the surface portion of the number three hoist structure. The worker fell through an open floor section, landing on the surface below and later died from complications related to the injuries.

In response to the fatal accident, safety authorities are highlighting critical hazard prevention protocols to protect surface and underground personnel from fall risks.

Mine operators must provide and maintain safe access across all active work areas. Any openings located above, below or near travelways through which workers could fall must be protected using properly installed and maintained guardrails, physical barriers or secure covers.

Where fall hazards exist, operators are reminded to enforce the use of safety belts, lanyards and fall protection gear equipped with reliable fall-arrest connections and secure anchorage systems. Thorough workplace examinations and comprehensive safety training remain vital to ensure personnel maintain situational awareness near overhead or open-floor hazards.
