The July 30, 2016, crash in Lockhart, Texas, killed the pilot and 15 passengers. The board's Dec. 9 hearing will examine the circumstances of the accident and the larger issues involving the safety of hot-air ballooning, including commercial balloon operations, training and decision-making, regulation and oversight, and medical factors.
The willful violations relate to the employer's failure to have an implemented, written bloodborne pathogens exposure control plan, including performing an exposure determination and offering exposed employees the Hepatitis B vaccine.
The recommended practices are flexible and can be adjusted to fit small and large construction companies handling short-term or multi-year projects, according to OSHA.

The City of Bend Fire Department is getting a $30,710 grant to launch a training project designed to reduce injuries and claims costs among firefighters by improving the efficiency of their physical movements and their flexibility and core strength.
The inspection began May 16, 2016, after a complaint that an amputation injury early this year at the window manufacturer's Dalton, Pa., facility was not reported to OSHA. The inspection also was done as part of OSHA's national emphasis program on amputations.
The grants will support tuition-free programs for industry-specific skills.
