A British electrical equipment supplier has been fined approximately $5.3 million after a worker suffered severe brain and spinal injuries when an unsecured 509-pound load fell from a forklift.
The 40-year-old worker was acting as a spotter during a loading operation at City Electrical Factors Ltd. in Telford, England, on Oct. 4, 2024. He required lifesaving surgery and now lives in a care facility, unable to communicate or care for himself.
An investigation by Britain's Health and Safety Executive (HSE) found the company failed to conduct a risk assessment, develop a lifting plan or properly supervise the operation. Workers also received no instructions for the task, despite performing it regularly.
HSE said the load could have been secured using existing anchor points or lifted more safely with the truck's crane.
The company pleaded guilty to violating Britain's workplace safety law and was fined Oct. 16, 2026.
HSE offers guidance on planning and organizing lifting operations.
