CSB Releases Factual Investigative Update on Loy Lange Pressure Vessel Rupture

The incident killed four people and left another in critical condition

The CSB has released a “Factual Investigative Update” on the April 3, 2017, rupture of a pressure vessel at the Loy Lange Box Company that killed four people and left another in critical condition.

The examination identified a history of leaks in the vessel, including in 2012 when a repair discovered water was leaking from the bottom of the tank.

After the incident, investigators found metal in the rupture had thinned well below its original state. The sudden mechanical integrity failure of the ring was cited as the immediate cause of the incident.
