At the sites of 63% of the 60 industrial professionals in Teamforce AI's Frontline Risk Study 2026, fewer than one frontline worker in 20 submits a safety, hazard or near-miss report each month, or the rate isn't tracked. That share combines 38% who estimated monthly participation below 5% with 25% who said their sites don't measure it. Of the 23 respondents below 5%, 19 said the rate hasn't changed in the past year.
Among respondents whose sites track reporting, 15 of the 30 using an app or EHS software said participation tops 15%, while none of those without the tools did. Of the 39 sites with either tool overall, 20 reported participation under 5% or did not track it.
Checking whether fixes worked was named a top need by 36% of respondents at sites using corrective-action software and by 30% of those at sites without it. That gap was not statistically significant, the study said.
Released Oct. 29, the study drew on a voluntary survey of 60 professionals conducted from July through September. Results reflect estimates made by the respondents themselves and point to associations rather than causes. Jim Lewis, executive director of the University of Iowa's Vaughan Institute of Risk Management & Insurance, partnered on developing the study.
