Wisconsin Beauty Manufacturer Fined After Chemical Explosion

OSHA proposes $275,694 in penalties for SV Labs following a nitrocellulose blast and fire at its Prescott facility.

Federal safety investigators have cited a Wisconsin beauty products manufacturer for multiple safety failures following a chemical explosion and a subsequent fire at its Prescott facility.

OSHA launched an inspection of SV Labs Prescott Corp. after a 55-gallon drum of nitrocellulose exploded. The blast triggered a secondary fire involving various flammable liquids stored on-site.

Investigators determined the California-based company failed to implement a comprehensive safety program for new hires. According to OSHA, the employer did not provide essential training on chemical hazards or respiratory protection. Additionally, the company failed to conduct required medical evaluations and fit testing for employees using respirators.

OSHA issued citations for two repeat violations regarding hazard communication and respiratory protection. The agency also noted 10 serious violations involving the improper handling of flammable liquids, lack of fire extinguisher training, and electrical hazards. Further citations were issued for forklift safety and recordkeeping discrepancies.

“Total proposed penalties for the violations reach $275,694,” OSHA officials stated.

SV Labs Prescott Corp. has contested the findings before the Occupational Safety and Health Review Commission. Under federal law, employers have 15 business days from the receipt of citations to comply, request an informal conference with OSHA, or contest the results.
