OSHA has fined Environmental Remediation Services a total of $142,200 for violations.
A new scoring system addresses these categories: structural integrity, safety, maintainability, reliability, fuel economy, emissions, noise, and performance.
Samuel S. Edwin, Ph.D., will become the new director of the division.
"The choices facing Metro about its headquarters building are clear: Either expend millions to retrofit the existing building to bring it up to modern fire and life safety standards or sell the building at the top of its potential value range and use those proceeds to fund a relocation," explained GM/CEO Paul J. Wiedefeld.
OSHA issued the company 12 citations for safety failures.
The airline's statement says the company expects an eight-hour network-wide delay and that more information will be available on the Emirates website and social media channels. It then listed 21 flights out of Dubai on Aug. 3 that have been cancelled as a result of the incident.
Investigators found the boat was not carrying a distress communication device (and it was not required to have one on board). Previous TSB investigations have found that carrying an emergency position-indicating radio beacon can help to save lives, and the board has recommended that small fishing vessels carry one or other similar equipment.
The overall respirable dust standard in coal mines is reduced from 2.0 to 1.5 milligrams per cubic meter of air.
The worker fell at a job site where OSHA previously cited violations. Material Handling Systems/MHS Technical Services faces total proposed penalties of $320,400.
