OSHA has cited Lunda Construction Company for one willful and five serious safety violations and placed the company in its Severe Violator Enforcement Program.
The company faces $198,550 in fines for the alleged violations.
The agency claims the builder's failure to follow safety standards caused the fatality.
The exercise is intended to improve Canada's response capabilities for a train derailment involving flammable liquids -- the scenario that occurred in July 2013 in Lac-Mégantic, Quebec.
"In 2015, 28 miners lost their lives, the lowest number of deaths ever recorded in a single year.... I am proud to be part of this institution and its legacy," Assistant Secretary Joseph A. Main wrote in a blog post honoring the agency on its 38th birthday.
May 2 is the new deadline for comments on the agency’s Guidance on Data Evaluation for Weight of Evidence Determination, designed to help employers consider all available information when classifying hazardous chemicals.
The natural gas explosion flattened two buildings.
OSHA issued the company two repeat citations and proposed penalties of $92,400.
The managers retaliated after employees raised safety concerns at the Selma foam manufacturer.
"Daylight saving time is a great time to remember to change your smoke alarm batteries after you set your clocks ahead. This small step is an easy one, but it can help save lives," said Jenny Fouracre, a spokesperson for the pizza company.
"This is a case [where] a minor repair could have prevented workers at this refinery from exposure to a life-threatening acid," said Cal/OSHA Chief Juliann Sum. "These citations and penalties are a wake-up call that refineries must follow strict safety protocols to protect their employees."
The conference will take place in Sugar Grove, Illinois.

The study will assess how well the Behavioral Analysis and Safety Improvement Category (BASIC) safety measures used by the Compliance, Safety, Accountability Safety Management System identify high-risk carriers and predict future crash risk, crash severity, or other safety indicators for motor carriers, including the highest-risk carriers.
