OSHA Cites Nebraska Biofuels Company After Fatal Explosion
OSHA cited a Nebraska biofuels facility for willful and serious violations following a deadly explosion, highlighting ongoing risks tied to combustible dust and equipment hazards.

OSHA has cited Horizon Biofuels Inc. for willful and serious workplace safety violations following a deadly explosion at the company’s Fremont facility in July 2025.
OSHA opened an investigation after the blast caused fatal injuries to a worker. The agency identified multiple hazards at the site, including combustible dust buildup, failure to safeguard equipment from ignition sources, and the absence of proper fall protection for employees working at heights greater than 4 feet.
OSHA proposed $147,542 in penalties for the violations. The company has 15 business days from the date of receipt to comply with the citations, request an informal conference with OSHA, or contest the findings before the Occupational Safety and Health Review Commission. Penalty amounts and citation details could change as the case progresses.
The incident underscores the ongoing risks posed by combustible dust and other industrial hazards when proper safeguards are not maintained. Employers are reminded to address recognized hazards and ensure compliance with workplace safety standards to protect employees from preventable injuries and fatalities.
