Wisconsin Metal Fabrication Company Cited After Teenaged Worker Dies

Sixteen serious and one other-than-serious safety and health violations were given to G.D. Roberts & Co. Inc.

After a 17-year-old worker was killed by a machine at a Columbus, Wis., metal fabrication facility, OSHA has issued G.D. Roberts & Co. Inc. 16 serious and one other-than-serious safety and health violations.

An investigation determined the worker was clearing scrap below a loading table for a laser-cutting system when the machine lowered onto the victim. The company failed to make sure proper lockout procedures were followed and did not train its employees properly, says OSHA.

Other citations were given for failure to conduct noise monitoring, provide audiograms, train workers on noise hazards, following respiratory protection standards, evaluate for airborne hazards, and more.
