Roofing Contractor's Safety Failures Led to Worker's Death and Another's Severe Injuries: OSHA

OSHA has determined the incident was preventable. Proposed penalties total $63,900.

One man was killed and another was severely injured when an aerial lift lurched forward, pinning and killing one worker while a second was able to escape the basket with multiple fractures in his leg, according to OSHA. The two workers were employed by Tecta America Southeast LLC and were rehabilitating a Ford dealership in Orange City, Fla., engaged in removing rain gutters when the incident occurred.

In response, OSHA issued one repeated and four serious safety citations to the company. Proposed penalties total $63,900.

"Tecta America Southeast could have prevented this tragedy by simply inspecting the lift before allowing its workers to use it," said Brian Sturtecky, director of OSHA's Jacksonville Area Office. "This company must immediately address safety hazards at its work sites and be vigilant in the future to protect its employees from harm."
