This is the sixth time the roofing contractor has been cited for exposing workers to fall hazards, according to OSHA, which issued $101,121 in penalties for the safety violations to Home Live Roofing LLC.
The company has been placed in OSHA’s Severe Violator Enforcement Program.

The Department of Housing Preservation and Development imposes a fee of $200 per inspection if it has to perform three or more inspections at the same location within the same heat season for heat violations or calendar year for hot water violations. During the 2015-2016 heat season, it billed for $148,800 in inspection fees.
"The whole system, from training through to fixing faults, was not strong enough to stop a series of errors by staff when working with people on the ride," according to HSE, which reported that the company made technical improvements to the ride and changed its systems afterward.
Inspectors found the company did not implement engineering, work practice, or respiratory protection controls and exposed about 40 employees to lead.
The employee was initially hospitalized with a body core temperature of 108 degrees.
The $117,740 in penalties includes a willful violation with the maximum allowed penalty of $70,000 for not performing regular safety inspections and not fixing defective items such as broken horns and broken seat belts.
