Had the flammable vapor cloud ignited, this could have resulted in multiple fatalities," HSE inspector Stephen Hargreaves said. "This incident highlights the need for all duty holders to implement and address all concerns and potential risks which have been identified. Tata's failure to do so in this case put a number of workers at risk of serious harm."

The agency's Board of Directors approved amendments to the Occupational Health and Safety Penalty Amounts Policy D12-196-6, and the changes were effective July 4, 2017.
The standards will help improve the customer experience.

The new requirements require nicotine-containing products or their packaging to be child-resistant and tamper evident, ban certain ingredients, include new labeling requirements and warnings, and also require all e-cigarettes and e-liquids be notified to MHRA before they can be sold.
