The contractor was cited for violations related to fall protection in 2019 and 2022.
The worker was terminated after reporting and having conversations about safety concerns and filing a complaint.
Obstructed exit routes and aisles were found at the location, the agency said.

One employer faces about $124,780 in proposed penalties while the other faces $53,574.
Comments on “Principles for Measuring the Impact of Voluntary Consensus Standards on Human Health and Safety” are due by June 15.

The employee, who had told supervisors about the injury “and applied for workers’ compensation benefits” in 2019, was fired.

At two stores in Rhode Island, OSHA found boxes stacked in an unsafe manner among other hazards.
OSHA proposed a combined total of over $550,000 in penalties after inspecting two stores last year.
The company faces citations for one willful and one repeat violation.
Under the memorandum signed in February, OSHA can now “issue certifications in support of applications for U nonimmigrant status and T nonimmigrant status visas,” the agency said.
The contractor faces citations for four serious violations and over $50,000 in proposed penalties.
In the last five years, employees have been found exposed to trench hazards seven times, OSHA said.
According to OSHA, the contractor allegedly “ignored safety requirements to save time.”
The state plan covers nearly 80,000 state and local government workers in Maine.
