HMI Industries Inc., of Strongsville, Ohio, has agreed to pay $400,000 as a civil penalty to the U.S. Consumer Product Safety Commission.
The Gateshead Metropolitan Borough Council and a tree surgeon pleaded guilty in the case, which involved an errant attempt to remove two trees near railroad tracks in January 2012.
A compliance inspection determined that Always Safe Fire Extinguisher and Safety, LLC marked high-pressure compressed gas cylinders but lacked the requisite testing equipment and could not provide evidence to demonstrate the cylinders were properly requalified.
The agency announces the results of the May impact inspections, which include 172 citations, 21 orders and two safeguards.
The agency has scheduled an informal meeting to discuss efforts to improve the Nationally Recognized Testing Laboratory Program.
An oncology practice in Kentucky has pleaded guilty to purchasing and selling unapproved chemotherapy drugs.
The agency has cited Industrial Insulation Group LLC for allegedly failing to protect workers from caustic chemicals, among other violations.
The DOT agency reported that its safety investigators found the company failed to ensure that its buses were systematically inspected, repaired, and maintained.
The department determined four of the violations stemming from a February 2013 incident were Severity Level I and one was a Severity Level II violation.
The slight decline "is a positive sign for the state" after an 11 percent increase in vehicle thefts during 2012, according to the law enforcement agency.
Three cases announced in the past week involved temporary workers allegedly exposed to hazards or hurt on the job.
"Arguably it is one of the best pieces of legislation on the statute books, although we know it is often misunderstood and misinterpreted. It has protected millions of British workers and driven sharp reductions in incidents of occupational death, serious injury, and ill health," HSE Chair Judith Hackitt said.
The change in Lubbock's status from a district to an area office includes added staffers who will focus on the safety and health of high-risk construction workers, with an emphasis on Hispanic construction workers.
The regional program is an effort of OSHA's Philadelphia Area Office. In the larger Philadelphia Region, 47 fatalities resulted from tree trimming and clearing from 2009 through 2013, according to the agency.
Beginning Aug. 23, 2014, motor carriers and drivers can request the violations be removed from the agency's Motor Carrier Management Information System if the driver was found not guilty or the violation was dismissed in court.
The state agency's citation includes one willful violation and two serious violations with total penalties of $67,500.
"The remedy imposed for SeaWorld's violations does not change the essential nature of its business. There will still be human interactions and performances with killer whales; the remedy will simply require that they continue with increased safety measures."
The agency has cited Pan-Oceanic Engineering Co., an engineering company, for allegedly failing to protect workers from construction hazards.
The fatalities occurred on construction sites May 18-21 and are still under investigation. The goal of the inspections is to "raise awareness for everyone working in construction that hazards can be identified and corrected," said acting Cal/OSHA Chief Juliann Sum.
"Our message to drivers is simple: If you're not buckled up, we'll see it. And we'll give you a ticket every single time," Acting NHTSA Administrator David J. Friedman warned.
