"It is a serious issue. It's the number of serious violations that were found. It means that many employees were at risk of serious injury," Norman R. Deitch said.
"Management continues to allow workers to clean equipment without following safety procedures and without guards being properly installed. This company needs to address all workplace hazards, not just the ones for which penalties have been proposed," said Bill Fulcher, director of OSHA's Atlanta-East Area Office.

Quality Blow Molding did not report the amputation, which is required by OSHA regulations.
The document laying out the procedures for implement the program includes a list of industries with high numbers and rates of amputations, as reported to BLS. These range from machine shops to sawmills, retail and commercial bakeries, meat processing, and manufacturing various types of food.
Case Farms Processing Inc. faces $861,500 in penalties and has been added to OSHA’s Severe Violator Enforcement Program.
Administrator Joseph A. Main has said that the reforms have been "real game changers."
"Kehrer Brothers Construction brought non-English-speaking workers to the U.S. and knowingly exposed them to asbestos," said Assistant Secretary Dr. David Michaels. "Kehrer also threatened to fire his employees if they spoke with our investigators. This is outrageous, illegal behavior. We at OSHA will do everything in our power to ensure this employer stops endangering his employees."
OSHA has cited Republic Steel for three repeat and eight serious violations.
Wilmer Cueva and Alfonso Prestia, respectively Sky Materials Corp.’s foreman and Harco Construction LLC’s senior superintendent, are charged along with their companies with recklessly causing the death of Carlos Moncayo, a Sky employee, in April 2015.
Maine becomes one of five states that manage their own health and safety programs programs for state and local government employees as approved by OSHA.
