Nebraska Railcar Cleaning Services has been cited for more than 30 violations after incident. The company faces penalties of $963,000, and OSHA has placed it in its Severe Violator Enforcement Program.
Harco Construction and Sky Materials Corp. were cited for willful safety violations. Proposed fines totaled $280,000.
"The tragic loss of two of Kansas City's firefighters is a reminder that our emergency response teams put their lives on the line for all of us every day," said Mayor Sly James. "Our condolences go out to the loved ones of those who made the ultimate sacrifice and we pray for recovery of the injured."
The goal of any safety program is to change behavior and it takes time for employees to fully adopt new safety skills.
"The investigation was not concerned with question of blame or liability. Answering those question is a matter for the criminal investigation," according to the board.

Robert Lytle of Rapid City, S.D., marketed the QLaser devices with labeling claims they could be used for treating conditions such as cancer, HIV/AIDS, and diabetes. "Although two of the devices are cleared for providing temporary relief of pain associated with osteoarthritis of the hand, none of the devices have been cleared by the FDA or otherwise approved to treat any other medical conditions," according to DOJ.
OSHA has cited Apache Mills Inc. for nine safety violations.
Ambrosia Fragrance of Piscataway, N.J., has been fined more than $67,000 by the agency.
