All Phase Roofing, Lennar Homes Faces OSHA Fines for Lack of Fall Protection

The companies failed to secure safety measures for roofers installing roofing membrane.

Following a recent investigation, OSHA has cited All Phase Roofing Inc. and Lennar Homes LLC for exposing workers to fall hazards at a worksite in Boca Raton, Florida. On June 28, 2023, OSHA officials witnessed a lack of fall protection measures for All Phase Roofing employees installing roofing membrane on the roofs of garages attached to two residential structures.

According to a release, All Phase Roofing faces three repeat violations, including the failure to take fall protection measures, neglecting inspections by a competent person to ensure proper safety equipment usage and allowing roofing work without proper hazard recognition training. 

In addition, All Phase Roofing received three serious violations for unsafe practices, such as using an interior staircase without a rail, employing an extension ladder improperly and lacking competent person training on ladder use. OSHA has proposed $159,117 in penalties.

Lennar Homes, the home builder associated with the worksite, received one serious violation for failing to have a competent person inspect the worksite to ensure employee safety equipment and stairwell railings were in placem, resulting in $8,929 in OSHA fines.

“Falls in construction is still the number one killer of workers, and it is irresponsible for All Phase Roofing to allow its employees to work on roofs unprotected. These types of hazards are well known, yet we still find employers subjecting their workers to dangerous and life-threatening work conditions,” OSHA Area Office Director Condell Eastmond in Fort Lauderdale, Florida, said in a statement. “General contractors also have a responsibility, as Lennar Homes did here, to take the necessary steps to ensure that its subcontractors maintain safe worksites.”

Since 2012, OSHA has collaborated with the National Institute for Occupational Safety and Health (NIOSH) on the Fall Prevention Campaign to raise awareness of common fall hazards and prevention methods.
