OSHA Fines Pennsylvania Manufacturer for Workplace Safety Hazards After Employee Amputation

Champion Modular Inc. faces $687,650 in penalties.

OSHA has cited Champion Modular Inc. for exposing employees to health and safety hazards at its Strattanville, Penn., facility. Champion Modular faces $687,650 in penalties.

OSHA began an inspection after an employee of the company suffered an amputation in November 2018. The agency issued serious and willful citations to Champion Modular Inc. for failing to use machine guarding, provide fall protection, and train employees on hazard communication and hearing conservation.

“Moving machine parts have the potential to cause severe workplace injuries if they are not safeguarded,” OSHA Erie Area Office Director Brendan Claybaugh said. “Employers’ use of machine guards and devices is not optional. Employers are legally responsible for ensuring that machine operators are protected.”

The company has 15 business days from receipt of the citations and proposed penalties to comply, request an informal conference with the area director of OSHA, or contest the findings before the independent Occupational Safety and Health Review Commission.
