NTSB Issues Reports on Four Maritime Investigations

One of the reports involves a 2016 Hudson River tugboat sinking.

The NTSB has released four marine accident briefs, including one on the 2016 sinking of the tugboat Specialist in the Hudson River. The agency has determined inadequate manning and the resultant crew member fatigue caused the incident in which three crew members died.

The agency said crew member aboard the ship had likely not received more than four to five hours of sleep in the three days prior to the accident. Bad weather, strong currents, and restricted visibility also contributed.

Other reports include: the March 2016 allision between tugboat Kodiak and the North Landing Bridge in Chesapeak, Va.; a January 2016 fire in American Samoa; and a January 2016 allision in Mississippi.
