NTSB Issues Update on Dynamic International Airways Flight 405

The plane caught fire while taxiing at the Fort Lauderdale-Hollywood International Airport.

The NTSB released an update Oct. 3 on Dynamic International Airways Flight 405, the Boeing 767 that caught fire on Oct. 29, 2015, while taxiing at Fort Lauderdale-Hollywood International Airport. The safety board found that the main fuel supply line coupling assembly had disconnected in the wing-to-engine strut above and behind the left engine. This piece is being retained for further inspection. An examination of the left engine found no evidence of engine-specific failure.

Fire did not penetrate the fuselage, but the left wing, left engine cowling, and left fuselage center section sustained thermal damage. According to the aircraft's records, the plane was in dry storage for 29 months before Dynamic International Airways leased the plane. The airline has operated the plane for 240 hours since leasing it.

Of the 90 passengers and 11 crew members on board, one was seriously injured and 21 sustained minor injuries. According to the board, Dynamic International Airways has issued a Fleet Campaign Directive to inspect the remainder of its aircraft to ensure proper installation of the fuel line coupling assemblies.
