The agency has previously reported that the initial explosion occurred in the refinery's Fluid Catalytic Cracking Unit at about 10 a.m. while the refinery was shutting down that unit for periodic maintenance and inspection.

The New York State Department of Environmental Conservation is overseeing the safe shutdown of operations at the facility. The coke battery consists of 60 ovens, of which 30 remained in operation at the time of shutdown; on Oct. 15 the last of the remaining 30 ovens were fully emptied, marking the end of coke production at the facility.
The Oct. 18 meeting concerns corrective actions the Tennessee Valley Authority agreed to make at its nuclear plants and corporate offices after NRC staff in 2016 expressed the concern that some operations employees may not have felt comfortable raising safety concerns at the Watts Bar nuclear plant.
Valid for 15 years, the approval means the commission finds the design technically acceptable, but it does not fully certify the design. NRC is preparing a rulemaking to fully certify the design for U.S. use.
"It is important for chemical and other facilities to be prepared for the unique safety challenges posted by cold weather," the agency's new Safety Digest document says, "such as hazards of water freezing and expanding, which can damage equipment or cause instrumentation to fail."
The program begins Oct. 1 with a three-month period of education and prevention outreach to encourage employers to bring their facilities into compliance with OSHA standards. Enforcement activities will begin after the outreach period and will continue through Sept. 30, 2019, unless it is extended.
