OSHA Cites MFA Inc. After Serious Fall
A farm and feed supplier received 13 safety violations after a worker was fatally injured in a fall.
OSHA has cited MFA Inc., an Aurora, Mo.-based farm and feed supplier, filing 13 serious safety violations against it after a worker was fatally injured in late August, according to the agency's news release. The worker died when he fell 40 feet from a manlift used to transport materials and landed on the facility's concrete first floor.
An OSHA investigation prompted by the accident found the company allegedly committee 13 serious violations of safety rules, including failing to guard open-sided floors throughout the plant with fall distances up to 20 feet, inadequate footing space on industrial ladders creating fall distances up to 125 feet, failing to inspect manlifts every 30 days, failing to have guarded belts and pulleys on machinery, failing to use proper connectors, and more. The company also allegedly failed to have two exit doors and had obstructed exits and poorly lit exit routes, as well as failing to have an alarm system to warn workers of emergencies.
The company faces $91,000 in proposed fines.
