Six Safety Violations Found at Silo Where Worker Died

The silo collapse occurred in December 2013 at a facility cited for several safety violations in June.

A silo collapse in Washington that left a worker missing and then presumed dead led to an investigation, which has found the facility was cited for six alleged serious safety violations in June, according to Fox News.

The accident occurred at a Wilcox Family Farms facility in Roy, Wash., when a silo collapsed and spilled 50 tons of corn on a worker. The worker was working with another employee performing maintenance on the silo. Rescuers did not find the entrapped worker's body until they were able to stabilize two other silos that were damaged in the accident.

The facility was previously cited for six safety violations totaling $10,000 in June, according to the Department of Labor and Industries. The safety violation cited the company for lack of adequate rescue and emergency services because employees who entered the confined spaces were at risk for death or injury because these emergency services were not available. The company was also cited for failing to inform employees of the hazardous conditions. According to Fox News, it is still unclear whether the site of the accident was part of the inspection done in June.

The company's owner, Brent Wilcox, issued a public apology saying he and his company are "deeply saddened by the loss of an employee."
