OSHA Cites Piramal Glass USA with 21 Violations Totaling over $130,000

The glass company receives the violations due to an investigation done after an employee was injured

After an employee lost a finger at a Park Hills, Missouri Piramal Glass plant in mid-March, OSHA inspected the facility and found 21 safety and health violations, according to a press release from OSHA. The man lost his finger while repairing a machine because prior to the fixing the machine, it was not isolated from its energy source.

The company faces a repeat violation—involving improperly mounting metallic receptacle boxes to a firm surface—as well as thirteen serious, safety-related violations. These include lack of e-stop devices on machines, failing to equip a lathe with a foot-operated guarding device, unguarded floor holes, missing railings, failure to complete periodic audits and more.

In addition to safety violations, Piramal Glass faces a handful of health violations, such as failing to develop and implement a noise monitoring program, exposing workings to high noise levels, failure to ensure the use of hearing protection, lack of personal protective equipment and more. Two other violations include not identifying machinery on audit records and not inspecting fire extinguishers.

Piramal Glass also runs facilities in Mays Landing and Williamstown, N.J., India, and Sri Lanka.
